Abstract: The Young Offender Project was funded by the Faculty of Health Sciences, University of Adelaide. This research was undertaken part time by researchers in the Department of General Practice over 5 months between July 2005 and December 2005. Preliminary work entailing an extensive literature review for this project was supported by a Department of General Practice, University of Adelaide, vocational scholarship in January 2005. In addition, an Advisory Group comprising representatives from secure care facilities, youth health and research supported this project. The purpose of the study was to examine current discharge planning practices for young offenders at youth training centres in South Australia with a view to improving offenders’ connection with primary health care services on discharge through the implementation of a multi-dimensional health assessment tool.
